Strong and Stable Kit

The Strong and Stable Kit provides tools to help you stay strong and prevent falls.

This kit includes:

  • A resistance band strength kit
  • Tip sheets with helpful fall prevention advice
  • Tub grips to make your bathtub or shower safer
  • Night-light to keep a bathroom or another area of your home well-lit at night
  • Medication box to help take medications correctly

Activity tracker plus Personal Emergency Response System (PERS) device

Aspirus Health Plan Essential Rx members are eligible to get an easy-to-use activity tracker plus PERS device that features:

  • 24/7 emergency call-for-help to a support agent directly through the watch
  • Step and heart rate tracking to help you reach your health goals
  • Built-in GPS to support you both inside and outside your home

It’s ready to use right out of the box. You don’t need to set it up or pair it to a cell phone or Wi-Fi. Members who use the device are also eligible for a blood pressure monitor.

Quit Smoking and Vaping Program

Learn how to stop smoking, vaping or chewing tobacco at no charge through the tobacco and nicotine quit line. Nicotine patches, gum or lozenges are also available to eligible members.

Mom's Meals

Stay healthy while recovering from a hospital stay with home-delivered meals. Mom's Meals helps by delivering healthy, balanced meals to your home so you can focus on recovering. Designed by professional chefs and registered dietitians, all meals are made with high-quality ingredients.

Eligible members with a chronic heart disease diagnosis on file with Aspirus Health Plan may receive up to 28 meals over two weeks after discharge from a hospital stay. To learn more, call the customer service number on the back of your member ID card.
